IMPORTANT NOTE:
1. Group Hospital & Surgical Coverage includes 24/7 Third Party Administration Service by Etiqa Healthcare to manage hospital admission and discharge.
2. Eligible age between 16 to 65 years old, age next birthday.
3. The Maximum limit for “Repatriation and Funeral Expenses” is RM7,000.00.
4. The above coverage is subjected to Etiqa policy terms, conditions and exclusions as specified in the policy contract and arrangement made between Etiqa Takaful and Universiti Pendidikan Sultan Idris.
WAIVER CLAUSES FOR GROUP HOSPITAL & SURGICAL PLAN:
5. Waiver on 30 days waiting period for new and existing member
6. Pre-existing illnesses condition waived for new and existing member.
7. Specified illnesses condition waived for new and existing member.
8. Waiver of 20% co-payment due to upgraded room and board.
ADDITIONAL BENEFITS / SERVICES:
1. Advances for Repatriation/ Funeral Services Expenses
The appointment of repatriation agent can be arranged by Etiqa Agency at no cost to University.
Etiqa Agency also agreed to advance the full amount of cost incurred to repatriate the deceased body back to their home country.
Scope of job for Repatriation Agent
The repatriation agent appointed will be responsible to handle all the processes until the mortal remain arrive safely in their home country
- To deal with the Embassy and Hospital in securing release of remains.
- To ensure all medical procedures, morgue procedures and registering death documents with the authority are completely done.
To provide all the necessary transportation arrangement including booking of flight.
2. Cardless Medical Care and 24/7 HealthCare Call Centre
Student can visit hospitals without the need for a physical medical card. Just present the passport and the hospital will verify with Etiqa HealthCare Call Centre.

5. Coverage for Dependents (Spouse/Children/Parent) for Visa Purposes
Immigration Department of Malaysia has imposed a compulsory requirement to have insurance policy for renewal of dependent visa.
Therefore, the student can contact us directly at 012-6144 052 to purchase the insurance policy either with medical/hospital coverage or just Personal Accident coverage only.
